The psychiatric evaluation consists of two parts:
1. Initial Assessment Appointment (60-90 Minutes) - $300
We will spend our initial appointment discussing your current symptoms, the ways they interfere with your goals, and how they have changed over time. We will also establish the foundation of your symptoms by forming a timeline of your life identifying turning points (good and bad) as they may inform our treatment options or illuminate aspects of your illness that would otherwise be overlooked. By the end of the appointment, I hope to have a good sense of who you were, who you are, who you hope to be, and what is getting in the way. Depending on your symptoms, I may request that you complete bloodwork or ratings scales after our appointment. During our appointment, I will ask you to sign consent forms for all the other people involved in your care (i.e. Therapist, Doctors, Family). Medication is not generally prescribed during this appointment.
2. Collaboration & Planning Appointment - ~2 weeks later (30-45 Minutes) - $150
In between our first appointment and the collaboration & planning appointment, I will contact the other people involved in your care to get their impression of your symptoms and brainstorm ways we can come together to provide you the best treatment possible. When you arrive for your appointment, I will review my impressions from our first appointment as well as the information I gathered from any bloodwork, rating scales, and other people involved in your care. This allows you the opportunity to correct anything I may have misunderstood and ask questions. I will offer several evidence based treatment options and help you to weigh the risks and benefits of each option. Once you are comfortable with our treatment plan, we will get started right away.
In person or telemedicine follow-up appointment - $150 per 30 minutes
Following the treatment plan we crafted during your psychiatric evaluation, we will meet every 1-3 months to discuss how your treatment is progressing. I will work with you to identify methods we can improve the effectiveness of your treatment such as apps on your phone or changes in your diet. Supportive psychotherapy techniques will be used to reinforce skills you have learned in therapy and maintain the gains you have made through treatment.
Insurance Policy: The practice does not bill insurance companies or participate in any insurance plans. Individualized, flexible, and private care means you can see me based upon our collaborative treatment plan, rather than decisions made by insurance companies and managed care organizations. Your insurance company can advise you about out-of-network benefits unique to your policy. At your request, I can provide a “super-bill” inclusive of diagnostic and service codes. Using this “super-bill,” you can submit claims directly to your insurance company for reimbursement. I do not participate in any insurance plans, including Medicare and Medicaid.
Good Faith Estimate: Prior to your first appointment, you may request a Good Faith Estimate. This estimate includes an individualized report estimating the anticipated costs of treatment for your condition.
